Abstract
Readiness to face discharge is a determining factor for the success of treatment while at home after hospitalization. Failure to prepare for discharge or the failure of the patient and family to understand the limitations or implications of health problems can make the patient's condition worse and increase the risk of complications and re-hospitalization. The purpose of this study was to identify the readiness of Type II Diabetes Mellitus patients in facing discharge at Budi Rahayu Hospital, Blitar City. This research is descriptive research. The sample in this study were 15 people with the sampling technique is purposive sampling. The inclusion criteria applied in the sampling process were Type 2 DM patients who would face discharge with the criteria for going home to recovery, Type 2 DM patients without complications, and cooperative patients. Meanwhile, the exclusion criteria included inpatients with a length of stay less than 4 days. This research was conducted in January 2020 in the Inpatient Room of RSK Budi Rahayu Kotar Blitar. Data analysis using frequency distribution. The results of this study indicate that most of the respondents, namely 11 respondents (73.3%), have a level of readiness for discharge in category 4. One method of increasing patient readiness in facing discharge is discharge planning. Discharge methods in a structured manner from admission to discharge by involving all disciplines that directly treat patients can increase knowledge, especially knowledge related to diabetes mellitus in a comprehensive manner.
